چکیده

BackgroundThe most appropriate treatment for displaced multiple-fragment proximal humeral fractures in elderly patients is currently unclear. Reverse total shoulder arthroplasty (rTSA) a promising option that being used increasingly. The purpose of this study was to compare the outcome rTSA vs. hemiarthroplasty (HA) 3- and 4-part patients.MethodsThis multicenter randomized controlled trial. We included aged ? 70 years with or between September 2013 May 2016. minimum follow-up period 2 years, measures including Constant score (primary outcome), Western Ontario Osteoarthritis Shoulder index, EQ-5D (EuroQol 5 Dimensions) range motion, as well pain satisfaction assessed on visual analog scale.ResultsWe 99 (48 patients) HA (51 patients). Fifteen were lost follow-up, leaving 41 43 analysis. mean age 79.5 there 76 women (90%). group had 58.7 points compared 47.7 group, difference 11.1 (95% CI, 3.0-18.9 points; P = .007). Compared patients, greater (79 mm 63 mm, .011), flexion (125° 90°, < .001), abduction (112° 83°, but no pain, index scores. identified 3 4 adverse events groups, respectively. Among 80 (n 38), (17 9 .17) (77 74 .73).ConclusionWe found provides better function than measured score, further emphasized by more satisfied their function. appears be mainly result motion (abduction flexion) group. results also indicate benefit less from 70-79 years. patients. Internal fixation versus shoulder hemiarthroplasty for displaced 4-part proximal humeral fractures in elderly patients.

The purpose of this single-center, randomized, controlled trial was to report on the 2-year outcomes of proximal humerus fractures in elderly patients treated with open reduction and internal fixation (ORIF) with either a locking plate or shoulder hemiarthroplasty. Thirty-two patients (87% women) with a mean age of 71.9 years (range, 67-86 years) were treated with ORIF with either a locking pla...

Hemiarthroplasty versus reverse total shoulder arthroplasty for acute proximal humerus fractures in elderly patients.

Proximal humerus fractures are the third most common fracture in elderly patients. Hemiarthroplasty has been the treatment of choice in patients with bone quality and fracture patterns not amenable to open reduction and internal fixation. Reverse total shoulder arthroplasty is a newer option that appears to be less dependent on tuberosity healing than hemiarthroplasty. The authors hypothesized ...

Humeral-sided Radiographic Changes Following Reverse Total Shoulder Arthroplasty

Background: We sought to characterize humeral-sided radiographic changes at a minimum of 2 years after reverseshoulder arthroplasty (RSA) to determine their association with specific implantation techniques. Methods: The immediate and most recent postoperative anteroposterior radiographs of 120 shoulders with primary RSAand a minimum of 2-years of radiographic follow-up were an...

Functional outcomes of reverse shoulder arthroplasty compared with hemiarthroplasty for acute proximal humeral fractures.

BACKGROUND Complex acute proximal humeral fractures may require prosthetic replacement of the proximal humerus. Reverse shoulder arthroplasty (RSA) has been suggested as an alternative to hemiarthroplasty in the management of such fractures. This study compared the functional outcomes of RSA with hemiarthroplasty in patients with acute proximal humeral fractures. MATERIALS AND METHODS All pat...

Reverse Shoulder Arthroplasty for the treatment of Proximal humeral fractures in the Elderly (ReShAPE trial) : study protocol for a multicentre combined randomised controlled and observational trial

BACKGROUND Proximal humeral fractures are common in older patients. The majority are minimally displaced and are associated with good outcomes after nonoperative treatment. Poorer outcomes are associated with displaced, multipart fractures. There is no clear benefit from surgical fracture fixation compared to nonoperative treatment. Replacement of the fractured humeral head with a hemiarthropla...
